Title: Locality Based Floating Support Services & Asset Based Community Integration Services Supporting Older People

Description: Bradford Council are seeking suitable supplier(s) to provide Housing Related Support, 7 days a week, 365 days per year subject to the individual Service Users support plan. This support will be provided on behalf of the Council to Services Users residing within the Council’s boundary, under a tenancy agreement, issued by a Housing Provider, and who would have assessed need and resulting support plan for Housing Related Support. The support plan will aid support workers and users alike to work towards achievable goals with a view to the person being able to successfully manage their own affairs and tenancy. In addition to these services The role of the Asset Based Community Integration service will be to provide support to communities to build social capital and develop the assets of the local area It will play a coordinating approach in ensuring older people, regardless of tenure, can get access to support, information, advice and activities that will help them to live healthily and well in their community. It will substantially focus on building the assets, skills and resources the community has to offer. The service will also provide a level of support to older people, whilst recognising that its main role is to enable communities to support themselves.
